Before You Book

1. Mobile appointments only
2. Please provide the complete service address when booking
3. Appointment availability is based on location, service area, and schedule
4. Please provide a valid phone number for appointment coordination
5. Required intake and consent forms must be completed before treatment
6. A telehealth clinical evaluation with our nurse practitioner is required before treatment.

Booking an appointment does not guarantee treatment. Your requested service is subject to clinical review and approval ##Your Appointment

Please ensure there is a safe, clean, accessible area where your treatment can be comfortably provided.

Following your clinical evaluation, the nurse practitioner will determine whether your requested service is appropriate and, when clinically appropriate, issue an individualized treatment order.

Southern Glow IV does not provide emergency medical services.

If you are experiencing a medical emergency, call 911 or seek immediate emergency medical care.

Cancellation / No-Show Policy

We understand that plans can change. If you need to cancel or reschedule, please contact us as soon as possible.

Cancellations or rescheduling with less than 24 hours’ notice, as well as missed appointments, may be subject to a $50 fee in accordance with Southern Glow IV’s Cancellation/No-Show Policy.
